LaVida Care is where to start.

Right here in your community, we provide welcoming and sympathetic HIV testing, prevention, and referrals to treatment – whether you can pay or not. These services are offered by San Fernando Community Health Center, which locals have trusted for almost 100 years.

LaVida Care is more than a program – it’s a family.

We want you to know that there are ways to protect yourself from HIV, and that through LaVida Care, you can get them. We can help you get started with HIV testing. There are medications available to defend against HIV infection if you’re at risk, or to prevent it if you think you’ve recently been exposed.

If you have HIV, we’re prepared to help you find treatment right away. You can live a healthy life with HIV, and LaVida Care is committed to helping you.

By joining LaVida Care, you’ll get access to all the services at SFCHC, like counseling, immunizations, and primary care. It’s another way we let you know how important you are as a member of our family.

Nothing should stand in the way of your health, least of all money.

LaVida Care, as part of San Fernando Community Health Center, accepts many different insurance plans. If you don’t have insurance, you can still get services on a sliding fee scale or through an assistance program.

Comprehensive Dental Services
Your SFCHC dentist is also your partner in helping you protect your teeth year-round. Of course, you know to brush your teeth regularly and floss daily, but they can suggest other good habits you may not know about. Keeping your teeth healthy can help keep you healthy.

Primary Care
Annual primary care visits are important because they are the best way to track any health changes and treat any new issues as early as possible.

Behavioral Health
The goal of SFCHC’s behavioral program is always the same: to help you get back to your life and feel like you again.

Chronic Medical Conditions

When you have the flu or a cold, you know that eventually with rest and some medicines you’ll recover and get back to normal. This is not the case with chronic illnesses. Many can interfere with a person’s life such as asthma, hypertension or high blood pressure, and diabetes. SFCHC can teach you how to maintain a positive physical, emotional, and mental outlook, while helping you create a long-term treatment plan to meet your specific needs.

The LaVidaCare program is funded by HRSA's grant for Los Angeles County (LAC), a priority jurisdiction of ENDING THE HIV EPIDEMIC - Primary Care HIV Prevention (PCHP). We receive funding from HRSA to increase PrEP accessibility, especially with underserved communities.
